Incidence of ES Beta L Producing Klebsiella pneumoniae among Inpatients and Outpatients from North-Eastern part of Karnataka

Background and objective: Extended-Spectrum beta-Lactamase (ESâL) producing bacteria may not be detected by routine disk diffusion susceptibility test, leading to inappropriate use of antibiotics and treatment failure. Current knowledge on antimicrobial susceptibility pattern is essential for appropriate therapy. Here we report the incidence and resistance pattern of ESâL producing Klebsiella pneumoniae clinical isolates recovered in Southern India. Methods: A total of 286 Klebsiella pneumoniae were isolated from1372 clinical samples.Antimicrobial susceptibilitywas determined by Kirby-BauerÂ’s disc diffusion method. All isolates were screened for ESâL production according the Clinical Laboratory Standards Institute. Results: Of the 286 K.pneumoniae isolates, 275 (96.15%) were positive for ESâL production. Multiple antibiotic resistances were most common and the majority of the isolates 129 (45.10%) were resistant to as many as seven antibiotics. All the isolates were sensitive to imipenam. Interpretation and conclusion: High percentage of drug resistance in ESâL producing Klebsiella pneumoniae suggests that monitoring of ESâL production and antimicrobial susceptibility testing are necessary to avoid treatment failure.
